Medical diagnosis is a process in which the physician makes a conclusion regarding the patient’s illness based on the physical examination of the patient, and from several medical tests. The tests are conducted in order to reach a conclusion about the right diagnosis, and usually include several basic examinations, such as measuring blood pressure, blood tests, pulse rate, urine tests, and electrocardiogram. During medical evaluation the doctor also obtains information from the patient himself, such as his previous state of health, and reads the history of diseases and medications registered in the computer.

The doctor makes a decision based on symptoms of the disease. Any sensations or changes in health function are considered symptoms. Understanding the significance of symbols can play a crucial role in understanding the nature of the patients’ illness. Some symptoms can be manifestations of different conditions. Some symptoms occur in a wide range of disease processes, whereas others are fairly specific for a narrow range of illnesses. The physician narrows down the possibilities of various diseases, and reaches a hypothesis regarding the illness likely to account for existing symptoms.

Many diseases are obvious; therefore most patient consultations are relatively brief. When there is doubt regarding the illness, another series of tests is performed, until the doctor reaches a qualified medical evaluation. Once the diagnosis is complete, the physician informs the patient of his illness, and proposes the right treatment. It usually includes therapy and follow-up, meaning further consultations and tests to monitor the condition and the progress of the treatment according to the guideline on the treatment of the particular illness.
