It will require getting approval from your doctor if your child is still be an infant, and you will also need to provide a copy of their birth certificate. If your child is in school, the form must also be assigned by the school principal as some states require you to do so.

Your responsibility as a parent is to also ensure that your child’s grades are not failing, so there must be a balance between their work and studies.
